Care Coordinator For Elders

Elder Advocate Role

We are seeking an experienced and compassionate Elder Advocate to join our team. As an Elder Advocate, you will be responsible for providing support to elderly individuals and groups, helping them access various services and resources. This role requires strong communication and interpersonal skills, as well as the ability to work independently and collaboratively with community programs and organizations.

Key Responsibilities:

• Evaluate the needs of elderly individuals and develop plans to provide support.
• Collaborate with community programs and organizations to ensure access to necessary services.
• Schedule regular visits and activities to promote socialization and well-being.
• Transportation service as needed.
• Report incidents of suspected elder abuse and neglect according to state law.

Working Conditions:
The incumbent will work in a variety of settings, including homes and community centers. Physical demands include standing, sitting, walking, and lifting up to 50 pounds occasionally.

Qualifications:

• High School Diploma or General Education Degree (GED)
• Experience working with elderly individuals and vulnerable adults
• Knowledge and experience working with cultural diversities

Special Skills:

• Ability to lift 30 lbs
• Basic computer skills
• Good oral & written communication skills
• Strong customer service skills
